The Forest West practice is in the shopping center with Food Town and Family Dollar; across the parking lot from Don Jose Mexican restaurant (same shopping center). The practice is directly next door to Subway.
There is a parking lot directly out front with ample free parking.
You can visit the practice by taking the METRO Bus Route #085, which runs on Antoine Dr. Get off at the stop on Antoine Dr @ Hardway Dr.
