How to get here

General
The Forest West practice is in the shopping center with Food Town and Family Dollar; across the parking lot from Don Jose Mexican restaurant (same shopping center). The practice is right next door to Subway.
Parking
There is a parking lot directly out front with ample free parking.

You can visit the practice by taking the METRO Bus Route #085, which runs on Antoine Dr. Get off at the stop on Antoine Dr @ Pinemont Dr.

+ What services do you offer?

We are a primary care practice designed for seniors on Medicare. We spend the time to listen to our patients to understand their health goals and concerns. 

Iora patients get care built on relationships and:

  • Convenient on-site labs so you don’t have to run all over town.
  • No rushed appointments that give you the time you need with your doctor and team
  • Same/next day appointments for current patients with immediate acute needs
  • Coordination of care with hospitals, specialists and your loved ones
  • $0 additional cost to your Medicare plan
+ What are the qualifications of your doctors?

All of our primary care providers are physicians and Board-Certified in Family Medicine or Internal Medicine. They are hired based on empathy and their experience working with older adults.

+ What happens if I’m sick and don’t have an appointment?

It’s important for you to know that we are here when you need us. We will see you same or next day if you are sick, and our physicians are available 24/7 via phone for urgent needs. We never want you to go to the ER or urgent care because you couldn’t get a hold of us. In addition to regular appointments, we can meet with you via phone, as well as communicate with you via email and text in our patient portal, Chirp, when you need it.

+ Are you affiliated with specific hospitals?

While we are not affiliated with a particular hospital or health system, so we can work with anyone that is in your insurance network, like Memorial Hermann, Methodist, MD Anderson and HCA.

We help our patients navigate the health care system and work with hospitals that provide them with exceptional care. We prefer to work with hospitals that are open to working collaboratively with us for the benefit of our patients. So, while we do work with everyone who is in-network with our patient’s medicare plan, we prefer to work with hospitals that are open to working collaboratively with us for the benefit of our patients.

+ What happens if I’m hospitalized?

If by chance you have to go to the hospital, we will be in direct contact with you and your care team. It is important to us that you know we are there for you. We even have a teammate, a Transition Navigator, who ensures that if you are hospitalized, we can follow you and make sure you’re getting the right care for you. Not only will we follow your progress and coordinate your care with the hospital team until you are discharged, after discharge we will initiate follow-up care as well. We also have access to many hospital electronic medical records.

+ Which insurances do you take?

We accept patients with traditional Medicare, with active Parts A and B, with or without most supplement/Medigap plans as well as Humana Medicare Advantage plans, including HMO and PPO. We can also work with Employee Retirement Services (ERS) and Teacher Retirement Services (TRS) Medicare Advantage plans offered through Humana. Our Patient Enrollment team can provide you with more information and discuss if we accept your specific Medicare plan.

+ Does Iora only accept adults on Medicare?

Yes! Iora is a primary care doctor’s office for seniors on Medicare.

+ What is a health coach and why would I need one?

At Iora, each patient gets a team dedicated to their care and each patient is paired with a Health Coach that works with them to determine their health goals, which they work to achieve together. 

Think of a Health Coach as your partner. They want to help make sure you are getting everything you need from Iora, your insurance, the health system, and the community. They can help you coordinate with specialists, family members, pharmacies, and more to help manage all aspects of your care. Learn more about why you need a health coach.

+ What is a first visit like? What do I need to bring?

We’ll start by getting to know you and learning about your health challenges and goals. We’ll make a plan for your care and set you up with any testing, medication or other care you might need. We recommend you bring your actual medication bottles to your first visit.

+ Do you work with all specialists? Can I keep my specialist?

Yes! You can keep your specialist. We can work with any specialist that is in your network, like UT Physicians or other specialists in private practice. If you already have an established relationship with a specialty doctor, your care team will coordinate care between them and us. Additionally, if you are in need of a new specialist, we have a list of local doctors that we work with and can get you the information for one that might work for you.

+ Are there any monthly/yearly fees?

There are no extra costs associated with being a patient at Iora Primary Care.

+ What does Iora stand for?

An Iora is a Sri Lankan bird, and Iora’s co-founder is originally from Sri Lanka.
