When you become a patient with Iora we help you navigate your way around your personal medical information via our collaborative care platform called Chirp. Chirp was built by Iora and is a handy application available on both your computer and/or phone’s web browser. The software will let you do a few things that will help improve your primary care experience:
Communicate with your care team
Nothing is more important to us than the sacred relationship between you and your care team. We work hard to make it as easy as possible to get in contact with your team. Whether you’d like to schedule an appointment or send an email or a text with questions, we have built the tools to keep you in contact with your team right within Chirp.
Learn about yourself
Let’s face it, health care is all about information. Through Chirp, we work hard to make sure you have access to all parts of your medical record so you have the information you need to understand your own health. Whether it is delivering your lab or x-ray results as soon as we get them, sharing the appointment notes with you as soon as we write them down, or sharing your medical records from your specialists, Chirp will keep you in the know.
Keep track of your care plan
Our patients can affirm that Iora’s care teams are fully committed to helping you see through your health goals. Because you’re the core component of that team, we recommend using Chirp as a means of tracking your care plan and health goal progress. Maybe you’re trying to lose weight and don’t have a record of your progress written down; Chirp can help you track that information and keeps it handy for you to check.
Chirp is constantly evolving to meet and even exceed the needs of our patients. Because of that, if you have any questions or would like to see something different in Chirp, please let your care team know. After all, this software was designed for you.
